- PLEASE PRINT CLEARLY
- Attach the form to the DVD entry
- Get it in the mail

Note: Double check ALL info and spelling before mailing. The information on winning entries printed in the Festival Program comes directly from the Entry Form.
Also Note: There is a new themed category this year for entries that focus on an message of PEACE. The entry can be any genre (i.e. PSA, Documentary, Music Video, etc.).
